The confidential information you are about to enter is required to initiate your first SCORE Counseling Session. Your first counseling session can be at our office weekdays between the hours of 9:00am and noon, or it can be at another time and place to be jointly determined by you and your counselor.  If you are able to meet at our office during our business hours, please note this in the comments box at the bottom of the Request-For-Counseling form. If you already have an appointment with a counselor, it is  VERY IMPORTANT  to specify the date and time of your initial meeting, as well as the assigned counselor's name in the comments box at the bottom of the Request-For-Counseling form. This will prevent duplicate counselors from getting assigned to your case.  Also, your browser settings must allow cookies in order for this application to work properly.
